The RFU recommends that all clubs / schools keep a record of all rugby injuries in a book. Details should include:
Date, time, location of incident
Name of ill / injured person
Details of incident
What first aid was given
Outcome
Name and signature of person dealing with incident.
For more serious incidents, the RFU should be notified via the following form:
Serious incidents requiring RFU notification are:
1. Death during or within 6 hours of rugby activity finishing
2. Injury resulting in player being admitted to hospital (not merely an A&E attendance)
